Experience: Minimum 2 years RN experience, preferably 5 years at the bedside.
License: Active RN License
Certifications: BLS

Must-Have: Current in-patient Case Management experience; experience with post-acute facility referral platforms such as NaviHealth or AllScripts.

Description:
The Nurse Case Manager is responsible for facilitating discharge planning.
Candidates should have current in-patient case management experience.
Experience with post-acute facilities is preferred.
Experience in a pediatric setting is not required, but case management experience is essential.
Familiarity with electronic health record systems such as EPIC is preferred.
Case management certification is also preferred.
The position requires no weekends or holidays.
There will be no on-call responsibilities.
Candidates may need to float to other areas as needed.
