Job Description

II. SUPERVISION

A.    Reports directly to the Acute Care Manager
B.    Reports indirectly to the House Supervisor and other managers covering the department.
C.    Delegates and directs assigned shift caseload team which may be comprised of LPN’s and CNA’s. 

IV.    GENERAL REQUIREMENTS

A.    Demonstrate commitment to the organizational mission, vision, and core values.
B.    Maintain clinical competency as required for Acute Care including but not limited to age-specific competencies relative to growth and developmental needs of all ages of patients.
C.    Knowledge of federal and state regulations related to nursing applications and relative practice area.
D.    Communicate effectively both verbally and in writing.
E.    Delegate, oversee, and evaluate the work of all acute LPN and CNA patient care staff members.
F.    Maintain confidentiality of patient information.
G.    Exercise professional judgment, courtesy, and professionalism in daily interactions with others.
H.    Demonstrates relationships management and influencing behaviors.

I.    Demonstrates knowledge of Melham Medical Center Compliance Program, Code of Conduct, and Confidentiality standards. 
J.    Serves designated role for emergency management plan response.  Follows instruction from Incident Command as appropriate.
K.    Demonstrates knowledge of risk management, quality review reports, Sentinel Events, near misses and non-punitive work environment concepts.

f.    Appropriately completes and/or supports reporting of events and forms as needed.
L.    Completes yearly self and staff evaluations/reviews and development plan.

N.    Attends 75% of monthly staff meetings and keeps updated on information.
O.    Completes annual on-going competency plan.
P.    Attends mandatory in-services and/or programs.
Q.    Maintains certifications.

V.    SIGNIFICANT DUTIES

A.    Initiates assessment/reassessment based on the patient’s needs per nursing standards and organization policy and procedures.
B.    Synthesizes pertinent patient data to develop nursing care plans and identify nursing priorities for care and desired outcomes.
C.    Collaborates with the interdisciplinary team to coordinate and provide patient care.
D.    Demonstrates knowledge and skill in nursing theory and ability to learn and develop competence in patient care across the lifespan.
E.    Demonstrates the ability to learn new practices and has the ability to practice in multiple settings within the rural hospital – medical/surgical, pediatric, neonatal, and skill care. 
F.    Integrates appropriate standards of care into the patient’s plan of care based on the diagnosis and age-specific needs.
G.    Executes, delegates and/or assigns other staff members to perform therapeutic and/or supportive interventions.
H.    Evaluate the patient’s response to and effectiveness of interventions.
I.    Evaluates the patient’s progress toward established outcome.
J.    Incorporates cultural considerations into the plan/delivery of care. 
K.    Includes family and/or significant others in the care of the patient as appropriate.
L.    Documents patient care and related information according to Melham procedures for documentation
M.    Facilitates a patient’s transition to the next level of care including admission, discharge, and transfers.
N.    Demonstrates knowledge of EMTALA regulation and paperwork.
O.    Assumes responsibility in a shared or delegated assignment and supports team member’s practice through timely communication.
P.    Services as a role model to colleagues, students, and other care providers.
Q.    Actively participates in continued learning opportunities and continued self-development pertinent to all areas of Acute Care nursing.
R.    Maintains effective communication channels with the entire healthcare team.
S.    Acts as a resource to the healthcare team
T.    Performs other related duties as assigned.
U.    Performs additional work duties during low volume periods when core staffing is in place.
