**Job Title:** Registered Nurse (RN)**Job Specialty:** Hospice**Job Duration:** Permanent**Shift:** 1:30 PM – 10:00 PM**Guaranteed Hours:** Not specified**Experience:** 2 years of RN experience required; Hospice experience strongly preferred**License:** RN license from Michigan required**Certifications:** BLS Certification required**Must-Have:** Reliable transportation and a valid driver’s license**Job Description:**- Work under the supervision of the unit’s Supervisor/Manager/Director, responsible for the delivery of patient care through the nursing process of assessment, planning, implementation, and evaluation.- Direct and guide patient teaching and activities of ancillary personnel while maintaining standards of professional nursing and policies.- Assess the physical, psychological, and social dimensions and educational needs of the patient on admission; complete a written assessment within the designated time frame.- Identify patient needs and formulate a written care plan based on the patient assessment; initiate the care plan within 24 hours of admission.- Implement the plan of care and document its effectiveness in the medical record, revising/ updating the plan as necessary or at least every 24 hours.- Establish a compassionate environment by providing emotional, psychological, and spiritual support to patients and their families.- Promote patient independence by establishing care goals, teaching self-care skills, and answering questions.- Assure quality of care by adhering to therapeutic standards, measuring health outcomes, and making or recommending adjustments as necessary.- Resolve patient problems and needs using multidisciplinary team strategies.- Protect patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.- Maintain continuity of care among nursing teams and physicians by documenting and communicating actions, irregularities, and continuing patient needs.- Maintain a cooperative relationship among healthcare teams by communicating information, responding to requests, building rapport, and participating in team problem-solving methods.- Utilize leadership skills through delegation, communication, and evaluation of team members.- Recognize changes in the status of the patient and provide appropriate intervention and documentation.- Provide ongoing patient and family education based on daily assessment of disease process, care requirements, and expected recovery.- Prioritize care based on individual patient needs.**Benefits:**- Competitive wages, salaries, and benefit packages, including 401K, paid time off, and flexible spending accounts.- Patient-centered work environment.- Discounts at the hospital café, gift shop, and area businesses.- Employee recognition programs.- Tuition reimbursement.- High levels of employee, leadership, and physician engagement.
