Must-Have:
Proficiency in HomeCare HomeBase (HCHB) and recent experience using it.
Proficiency in Oasis and recent experience using it.
A vehicle, valid driver’s license, and auto insurance.
Willingness to travel up to 75 miles from the designated care center.

Description:
Provides coordinated and directed home nursing care according to physician orders.
Applies nursing processes competently according to agency policies and procedures.
Responsible to the Clinical Manager.
Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Performs home visits and attends periodic office meetings.
Average number of patients per day ranges from 5 to 10.
Must travel between patient homes.
Mileage is compensated at the IRS standard rate for travel between patient homes.
Commute to and from the first and last patient is not billable.
Weekly productivity expectations are between 28 and 30 units.
All documentation must be completed within 24 hours of the appointment.
Declining patients may result in cancelation of the assignment.
There are no guaranteed hours; must meet productivity points to fulfill contract expectations.
