Description:
Coordinates and directs the provision of home nursing care according to physician orders.
Ensures competent application of the nursing process through agency policies and procedures.
Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses.
Must perform home visits and attend periodic office meetings.
Must have reliable transportation and a current driver’s license.
Agency-required liability insurance is also necessary.
Expected to visit approximately 5-10 patients per day.
Different activities such as documentation and travel hold various point values.
Contractor can discuss floating to nearby locations if patient census is low.
Types of cases include Start of Care, Wound Care, and Follow-up Visits.
Must accept every scheduled patient regardless of travel distance.
Documentation must be completed within 24 hours of patient appointments.
No guaranteed hours are provided with this contract.
Weekly productivity points expectations are set at 30 points for full-time hours.
Must be open to traveling up to 75 miles from base location.
Weekend rotations and on-call duties may be required every four weeks or less.
Must be proficient in EMRs currently used within the last six months.
First-time applicants in this specialty will not be accepted.
