Description:
Provides, coordinates, and directs the provision of home nursing care according to physicians’ orders based on agency policies and procedures.
This role requires the competent application of the nursing process.
The nurse is responsible to the Clinical Manager.
Directly supervises and evaluates Home Health Aides and Licensed Practical/Vocational Nurses.
The job involves home visits and periodic office meetings.
Must have reliable transportation, a valid driver’s license, and agency-required liability insurance.
Expected to handle 5-10 patients per day with an average of 5-8 patients.
Types of cases include Start of Care, Wound Care, and Follow-up Visits.
The nurse must be willing to travel up to 75 miles from the home clinic.
Mileage is reimbursed at the IRS Standard rate.
Locals are accepted at the same rate.
Typical schedule is Monday through Friday, 8:00 am to 5:00 pm.
There will be a weekend rotation of 1 weekend per month.
On-call is required during the weekend scheduled to work.
The position covers Sussex County with a travel expectation of 200-250 miles per week.
Hospital/agency highlights include a dress code of scrubs.
All documentation must be completed within 24 hours of the appointment with the patient.
