Job Title: Home Health Registered Nurse

– Experience in developing and communicating realistic care plans
– Knowledge of OASIS documentation requirements
– Familiarity with infection control standards
– Effective communication and teamwork skills

Description:
Utilize the nursing process to assess patient/caregiver needs to provide quality care to assigned patients in accordance with evidence-based practices and regulations.
Develop and communicate a realistic plan of care for each patient.
Serve as Case Manager for assigned patients and manage case loads and visit times according to productivity standards.
Effectively complete OASIS documentation per policies and regulations in a timely manner.
Implement medical plans of care, nursing interventions, and procedures to ensure the safe delivery of quality care.
Stay up-to-date on high-tech skills including IV use, wound care, and infection control standards.
Collaborate with LPNs to ensure adherence to the plan of care and delegate tasks as appropriate.
Evaluate various aspects of the implemented plan of care, including problems, care provided, and patient responses.
Maintain accurate documentation of nursing care provided to enable effective integration of information for ongoing quality of care.
Follow agency protocols for documentation processes and ensure timely completion of necessary forms.
Conduct patient and caregiver education to support informed decision-making.
Identify interdisciplinary needs of patients and communicate documentation to relevant professionals in a timely manner.
Assess patients’ needs for services and develop coordinated plans of care accordingly.
Report patient incident information to quality improvement specialists as necessary.
Coordinate safe patient discharges and transfers, ensuring all appropriate documentation is processed timely.
Engage in case conferences and peer review audits to maintain effective communication.
Advocate for patient care issues to ensure quality and sensitivity to individual needs.
Actively participate in interdisciplinary team meetings to assess and respond to patient needs.
Maintain proficiency in pain control and symptom management techniques.
Coordinate patient care with physicians, providers, and team members to enhance care effectiveness.
