Job Title: Registered Nurse Job Specialty: Case Management Job Duration: 22 weeks Shift: 9:00 AM – 5:00 PM, Monday-Friday (5×8 Hour Shifts) Guaranteed Hours: 40 hours per week Experience: Two years in case/resource management and utilization management required. 3-5 years in care management preferred. License: New York State RN License, Primary Source Verification Certifications: AHA BLS Must-Have: EPIC experience, PRI (Patient Review Instrument), and SCREEN experience/certification ideal Job Description: – Conduct comprehensive case and resource management duties. – Implement utilization management procedures efficiently. – Collaborate with healthcare teams to optimize patient care and resource allocation. – Utilize EPIC for documentation and data management. – Apply expertise in Patient Review Instrument and SCREEN assessments to support patient evaluations. – Adhere to established guidelines and protocols to ensure high-quality care management. – Maintain communication with patients, families, and healthcare providers.
