Description:
The RN Care Manager will oversee a daily caseload of patients in inpatient units with a variety of care needs.
The RN Care Manager will attend interdisciplinary rounds.
The RN Care Manager will complete initial assessments.
The RN Care Manager will facilitate the development of a plan of care in collaboration with the patient, family, caregiver, and healthcare team.
The RN Care Manager will communicate consistently with medical teams and patients regarding progress of care needs related to post-discharge care.
The RN Care Manager will interact with post-acute care referral sources to establish a plan.
The RN Care Manager will document and place orders for home health, durable medical equipment, and nutritional needs within the electronic medical record system.
