Job Description

Job Title: RN Case Manager
Location: Chillicothe, OH 45690
Shift: Day 5×8-Hours (8am – 430pm)

Job Type: Contract
Duration: 13 Weeks
Start Date: 05/26/2025

Key Responsibilities:

  • Navigating the swingbed needs/discharge planning
  • Facilitates transition planning across the care continuum by involing the patient, family, providor, and other healthcae team members. 
  • Works with patients/families to promote self- management support of chronic diseases through individualized plans with short/long term goals. 
  • Ensures suspected cases of abuse, neglect or exploitation have been referred/reported to the appropriate parties or agencies.

Required Qualifications:

  • Minimum of 3 year Case Manager experience
  • BLS Certification
  • Compact or OH State RN License
