Job Title: Pediatric RN – Home Care

Experience: One year of experience as a clinical pediatric nurse strongly preferred, home care experience preferred.
License: New York State Licensed RN
Certifications: ACLS, BLS

Must-Have:
Certified Home Health Care Experience Required
EPIC or Netsmart Preferred

Description:
Develop care plans appropriate to the patient and family’s health care needs in conjunction with the patient, family, physician, and clinical operations team.
Obtain medical orders and approval of medical services.
Complete the OASIS Assessment as needed to accurately reflect the clinical, functional, and social needs of the patient.
Ensure the completion of all appropriate clinical records needed for compliance with legislation and the delivery of seamless service within established time frames.
Refer to appropriate health care systems and community agencies/resources.
Communicate to health care team members the outcome of the assessment/reassessment to ensure appropriate follow-up based on the needs of the patient/family’s plan of care.
Display an exemplary level of patience, courtesy, and flexibility.
Interact with patients, physicians, and staff in a manner conducive to maintaining positive relationships.
Ensure problem resolution at the point of contact.
Maintain appropriate productivity requirements.
Adhere to Professional Home Visit Performance Standards.
Demonstrate strong interviewing, assessment, and communication skills.
Exercise initiative and independent judgment.
Build and maintain professional relationships.
Travel throughout the service area year-round.
Work independently with minimal supervision and adapt to change.
