Job Description

Job Title: LPN Part Time

Shift: Part Time Nights
Hours per Shift: 11 PM – 7:30 AM (2 shifts 8 hours one week, 3 shifts 8 hours the next week)
Experience: 1 year as an LPN
License: NY LPN Licensure
Certifications: BLS (AHA or RQI specifically)
Must-Have: High School Diploma or GED

Description:
Under the direction of licensed nursing personnel, performs direct and indirect designated resident care within the scope of practice as defined by certification.
Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients served.
Possesses the ability to assess data reflective of the patient’s status.
Interprets appropriate information needed to identify each patient’s requirements relative to age-specific needs.
Ensures that resident’s activities of daily living needs are met as outlined in the plan of care and in accordance with policies and procedures.
Meets residents’ hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, and skin care needs.
Completes documentation per protocol.
Maintains resident’s environment and safety as outlined in the plan of care and in accordance with policies and procedures.
Answers call lights and alarms promptly.
Provides safety mats and low beds, and uses restraints under the direction of nursing.
Maintains cleanliness of the unit and participates in behavior and fall prevention.
Utilizes and monitors equipment while ensuring it is not malfunctioning.
Documents completed tasks per protocol.
Ensures that resident’s nutritional needs are met as outlined in the plan of care and in accordance with policies and procedures.
Delivers meals, prepares trays, utilizes adaptive equipment, and assists with feeding and nourishments.
Honors residents’ preferences and choices.
Completes documentation per protocol.
Addresses resident’s rehabilitative (restorative and maintenance) needs as outlined in the plan of care and in accordance with policies and procedures.
Facilitates range of motion, ambulation, transfers, proper body alignment, and use of assistive devices.
Documents completed tasks per protocol.
Reports resident’s rights, refusals, and concerns to the Charge and/or Supervising Nurse in a timely fashion.
Identifies and reports changes in resident’s physical, emotional, and psychosocial status to the Charge Nurse promptly.
Occupational exposure to bloodborne pathogens is reasonably anticipated.
Tasks involve potential contact with blood, body fluids, or tissues.
