License: LPN Licensure

Certifications: BLS (AHA or RQI only)

Job Description:

– Provide direct and indirect resident care under the direction of licensed nursing personnel within the defined scope of practice.
– Ensure residents’ activities of daily living (ADLs) are met, including hygiene, grooming, dressing, and toileting needs within a safe environment, complying with policies and procedures.
– Maintain resident safety and environment, responding promptly to call lights and alarms, and engaging in fall prevention and equipment monitoring.
– Address residents’ nutritional needs, including meal delivery and feeding, while honoring resident preferences and documenting appropriately.
– Support residents’ rehabilitative needs, such as range of motion exercises, ambulation, transfers, and proper body alignment in accordance with care plans.
– Communicate residents’ rights, refusals, and any concerns to Charge and/or Supervising Nurse promptly, reporting changes in resident status as needed.
– Be aware of occupational exposure risks to bloodborne pathogens, handling tasks with potential contact with blood, body fluids, or tissues with care.
