Shift: Part-Time Evenings – Alternating Schedule (2 days one week, 3 days the following week) with Every Other Weekend and Holiday required

Experience: 1 year experience in an LPN capacity at a Long-Term Care Facility
License: NY LPN Licensure
Certifications: BLS (American Heart Association or Resuscitation Quality Improvement specifically)
Must-Have: Demonstrated knowledge and skills necessary to provide care appropriate to adult and geriatric patient groups
Job Description:
– Provide direct and indirect skilled nursing care for assigned residents.
– Follow defined policies and procedures under the supervision of a Registered Nurse.
– Maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff.
– Maintain knowledge and clinical expertise through continuing education and in-service programs.
– Collect data relevant to residents’ physical and psychosocial conditions on an ongoing basis and communicate findings to the RN and other members of the Interdisciplinary Team.
– Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions to achieve desired resident outcomes.
– Recommend revisions to the plan of care after evaluating resident outcomes and indicate the need for referrals to ensure all aspects of the resident’s care have been addressed.
– Collaborate with the resident/family and members of the Interdisciplinary Team within the scope of nursing practice, assuring customer satisfaction.
– Participate in the mentoring/orientation of staff and consider factors related to safety, cost, and impact in planning and delivering care.
– Under the direction of the RN, provide leadership in the professional practice setting and demonstrate knowledge and competency.
– Be aware that tasks involve potential contact with blood, body fluids, or tissues, with reasonable anticipation of occupational exposure to bloodborne pathogens.
