Must-Have:
– 1 year experience serving in an LPN capacity at a Long Term Care Facility
– Required to work weekends and holidays

Description:
The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit.
The individual must understand the principles of growth and development across the lifespan.
Possesses the ability to assess data reflective of each patient’s status and interpret the information needed to identify the patient’s requirements relative to age-specific needs.
Provides both direct and indirect skilled nursing care for assigned residents.
Follows defined policies and procedures under the supervision of a Registered Nurse.
Maintains collaborative relationships with other nursing staff, physicians, and ancillary staff.
Maintains an appropriate level of knowledge and clinical expertise through continuing education and in-service programs.
The LPN shall have the ability to provide care to geriatric residents.

Job Duties:
Collects data relevant to the residents’ physical and psychosocial conditions on an ongoing basis.
Communicates findings to the Registered Nurse and other members of the Interdisciplinary Team.
Contributes to the creation of a culturally relevant plan of resident care reflecting nursing diagnosis and interventions.
Involves the resident/family and healthcare team in the process while promoting a healthy, safe environment.
Recommends revisions to the plan of care after evaluating resident outcomes.
Indicates the need for referrals to address all aspects of the resident’s care.
Collaborates with the resident/family and members of the Interdisciplinary Team to ensure customer satisfaction.
Maintains current knowledge and competency in nursing practice.
Participates in the mentoring and orientation of staff.
Considers safety, cost, and impact when planning and delivering care.
Under the direction of the Registered Nurse, provides leadership in the professional practice setting.
Demonstrates knowledge and competency in practice.
Occupational exposure to bloodborne pathogens is anticipated.
Required tasks involve potential contact with blood, body fluids, or tissues.
