Job Title: Homestead Licensed Practical Nurse (LPN)

Job Specialty: Long-Term Care (LTC) LPN

Job Duration: 13 weeks

Shift: 2 PM – 10 PM, 5×8 per week, including every other weekend and holidays as required

Guaranteed Hours: Full-Time (40 hours per week)

Experience: Minimum 1 year as an LPN in a Long-Term Care Facility

License: LPN Licensure

Certifications: Basic Life Support (BLS) – AHA or RQI only

Must-Have: Experience in providing care to adult and geriatric patient groups

Job Description:

– Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse
– Maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff
– Continuously assess and collect data relevant to residents’ physical and psychosocial conditions, communicating findings to the RN and Interdisciplinary Team
– Develop and contribute to a tailored plan of resident care, reflecting nursing diagnosis and interventions, and involving resident/family and healthcare team
– Recommend revisions to the care plan based on evaluation of resident outcomes, ensuring all aspects of resident care are addressed
– Maintain up-to-date knowledge and competency in nursing practice through continuing education and in-service programs
– Support the mentoring and orientation of new staff, focusing on safety, cost, and care delivery impact
– Lead within the professional practice setting under RN direction, demonstrating knowledge and competency
– Manage occupational exposure to bloodborne pathogens, performing tasks involving potential contact with blood, body fluids, or tissues
