Description:
The Licensed Practical Nurse (LPN) coordinates and directs the provision of home nursing care according to physician orders.
The LPN will comply with agency policies and procedures through the competent application of the nursing process.
The LPN is directly responsible to the Clinical Manager.
The LPN supervises and evaluates Home Health Aides and other Licensed Practical Nurses.
Travel is required for home visits and periodic office meetings.
Reliable transportation, a current driver’s license, and liability insurance are needed.
The average number of patients per day is 5-8.
The contractor will be assigned between 5-10 patients per day, which equates to roughly 6-8 points.
Each type of visit and activity has different point values.
Types of cases include Start of Care, Wound Care, and Follow-up Visits.
Weekend requirements vary per location and must be discussed during the interview.
On-call requirements vary per location and must also be discussed during the interview.
The territory covers a radius of up to 75 miles, and mileage will be reimbursed at the IRS standard rate.
Local candidates are accepted at the same rate.
The contractor is expected to travel up to 200-250 miles per week.
Weekly productivity expectations are 35 units.
All documentation must be completed within 24 hours of the appointment.
There are no guaranteed hours for this contract; clinicians must hit weekly productivity points.
Declining patients may result in a cancellation of the contract.
Orientation will be provided.
