Pay Rate: $61.5-$66.5/hr
Shift: Days (5*8)
Start Date: Two weeks from the date of position offered.
Job Type: 13 weeks contract

Job Overview
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will have a strong background in medical terminology and case management, with experience in various healthcare settings such as Level I Trauma Centers, Primary Care, ICU, OR, Clinic, Emergency Medicine, and Discharge Planning.
Duties
- Provide comprehensive and holistic nursing care to patients.
- Coordinate patient care plans and ensure continuity of care.
- Conduct assessments and develop individualized care plans.
- Collaborate with healthcare professionals to optimize patient outcomes.
- Utilize utilization management techniques to ensure cost-effective care delivery.
Requirements
- Acute Hospital Experience by Specialty: ICU, MS, ER – Preferred
- Case Management/Utilization Review: Care Coordination, Discharge Planning, Needs Assessment/ Order DME – Required
- COMPUTERIZED CHARTING: EPIC – Required
- Additional Skills: Allscripts, Post acute care referrals – Required
- Years of Experience Required: 1
- Electronic Documentation: EPIC – Required
- Post acute discharge planning, authorization and referrals. No direct Utilization experience needed.
Requirements
- Minimum a year experience required.
- Active BLS required.
- Active RN-MI License required.
