Job Description

Job Title: LPN

Hours per Shift: 5×8 per week, every other weekend and holiday required
Experience: 1 year as an LPN
License: New York LPN Licensure
Certifications: BLS through AHA/RQI only
Must-Have: Experience in an LTC facility

Description:
The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served.
The individual must understand the principles of growth and development over the life span.
The LPN should possess the ability to assess data reflective of the patient’s status.
Interpret the appropriate information needed to identify each patient’s requirements relative to their age-specific needs.
Provide the necessary care for adult and geriatric patient groups.
Provides direct and indirect skilled nursing care for assigned residents.
Follows defined policies and procedures under the supervision of a Registered Nurse.
Maintains a collaborative relationship with other nursing staff, physicians, and ancillary staff.
Maintains an appropriate level of knowledge and clinical expertise through continuing education.
Participates in inservice programs to enhance skills.
The LPN must provide care to geriatric residents.

Job Duties:
Collect data relevant to the resident’s physical and psychosocial condition on an ongoing basis.
Communicate findings to the Registered Nurse and other members of the Interdisciplinary Team.
Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of care.
Reflect nursing diagnosis and interventions to achieve desired resident outcomes.
Involve the resident/family and healthcare team in the care process while promoting a healthy, safe environment.
Recommend revisions to the plan of care after evaluating resident outcomes.
Indicate the need for referrals to ensure all aspects of the resident’s care have been addressed.
Collaborate with the resident/family and members of the Interdisciplinary Team within the scope of nursing practice.
Assure customer satisfaction in the care provided.
Maintain current knowledge and competency in nursing practice.
Participate in the mentoring or orientation of staff.
Consider factors related to safety, cost, and impact in planning care delivery.
Under the direction of the Registered Nurse, provide leadership in the professional practice setting.
Demonstrate knowledge and competency in all required tasks.
Occupational exposure to bloodborne pathogens is reasonably anticipated in this role.
Required tasks may involve potential contact with blood, body fluids, or tissues.
