Submission Requirements

• 1 year or more CURRENT Dialysis experience – Required 
• Copy of Current, Valid Driver License – Required
• Geriatric patient experience – Required
• NXStage Dialysis Machine experience – Strongly required
• CNN or CDN Certification – PREFERRED 
• Crown Web EMR experience – Preferred
• BSN Preferred
