Travel Nurse RN – Case Manager in Fort Defiance, AZ

Primary Care Clinic at Tsehootsooi Medical Center.

Title: Care Coordinator (case manager) nurse positions within our Primary Care clinic:

Requirement:  Three (3) year Case Management Experience.

Shift: Weekdays Monday through Fridays from 8a to 5p, with One (1) hour lunch, no weekends or holidays. Collaborate with support nursing staff and with providers for patient care.

• Housing: Information can be found under the ‘Facility Group Document’ section.
• Orientation: on Mondays only – If your traveler misses the compliance deadline, they will be pushed to the following scheduled Orientation.

Travelers are occasionally expected to float to other departments in which they are qualified to work. This could include taking a patient load if experienced/competent in that setting or could be task-oriented such as answering call lights, phones, etc * Housing: Information can be found under the ‘Facility Group Document’ section in Medefis. * Charge Rates: travelers may be asked to work charge, no additional charge pay * Orientation: on Mondays only – If your traveler misses the compliance deadline, they will pushed to the following Monday
