Description:
The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.
The individual must demonstrate knowledge of the principles of growth and development across the lifespan.
They must possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements.
This includes providing care needed by adult and geriatric patient groups.
The LPN provides direct and indirect skilled nursing care for assigned residents.
They follow defined policies and procedures under the supervision of a Registered Nurse.
The LPN maintains a collaborative relationship with other nursing staff, physicians, and ancillary staff.
They ensure an appropriate level of knowledge and clinical expertise through continuing education and in-service programs.
The LPN shall have the ability to provide care to geriatric residents.
Job Duties:
Collect data relevant to residents’ physical and psychosocial conditions on an ongoing basis.
Communicate findings to the Registered Nurse and other members of the Interdisciplinary Team.
Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care.
Reflect nursing diagnosis and interventions to achieve desired resident outcomes.
Involve the resident/family and healthcare team in this process while promoting a healthy, safe environment.
Recommend revisions to the plan of care after evaluation of resident outcomes.
Indicate the need for referrals to ensure all aspects of the resident’s care have been addressed.
Collaborate with the resident/family and members of the Interdisciplinary Team within the scope of nursing practice to ensure customer satisfaction.
Maintain current knowledge and competency in nursing practice.
Participate in the mentoring/orientation of staff.
Consider factors related to safety, cost, and impact in planning delivery of care.
Under the direction of the Registered Nurse, provide leadership in the professional practice setting.
Demonstrate knowledge and competency in nursing practice.
Occupational exposure to bloodborne pathogens is reasonably anticipated.
Required tasks involve potential contact with blood, body fluids, or tissues.
