Description:
The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.
The individual must demonstrate knowledge of the principles of growth and development across the lifespan and possess the ability to assess data reflective of the patient’s status.
It is essential to interpret the appropriate information needed to identify each patient’s requirements relative to his or her age-specific needs.
The LPN will provide the care needed for the adult and geriatric patient groups.
Responsibilities include collecting data relevant to the resident’s physical and psychosocial condition on an ongoing basis and communicating findings to the Registered Nurse and other members of the Interdisciplinary Team.
The LPN will contribute to creating a developmentally age-appropriate and culturally relevant plan of resident care.
This plan should reflect nursing diagnoses and interventions to achieve desired resident outcomes, involving the resident, family, and healthcare team in the process.
Revisions to the plan of care will be recommended after evaluation of resident outcomes, indicating the need for referrals to ensure all aspects of the resident’s care are addressed.
Collaboration with the resident, family, and members of the Interdisciplinary Team will occur within the scope of nursing practice while ensuring customer satisfaction.
The LPN will maintain current knowledge and competency in nursing practice and participate in mentoring and orientation of staff.
Factors related to safety, cost, and impact will be considered when planning the delivery of care.
Under the direction of the Registered Nurse, the LPN will provide leadership in the professional practice setting.
The position involves occupational exposure to blood-borne pathogens.
Tasks may involve potential contact with blood, body fluids, or tissues.
