Description:
The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served.
The individual must demonstrate knowledge of the principles of growth and development across the life span.
The LPN should possess the ability to assess data reflective of the patient’s status and interpret the information needed to identify each patient’s age-specific requirements.
The role involves providing direct and indirect skilled nursing care for assigned residents.
The LPN will follow defined policies and procedures under the supervision of a Registered Nurse.
Maintaining collaborative relationships with other nursing staff, physicians, and ancillary staff is essential.
The LPN is expected to maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs.
The ability to provide care to geriatric residents is crucial.
Key responsibilities include:
Collecting data relevant to residents’ physical and psychosocial conditions on an ongoing basis and communicating findings to the RN and other members of the Interdisciplinary Team.
Contributing to the creation of a developmentally appropriate and culturally relevant plan of care.
Involving the resident and family in the care process while promoting a healthy, safe environment.
Recommending revisions to the plan of care after evaluating resident outcomes and indicating the need for referrals.
Collaborating with the resident, family, and members of the Interdisciplinary Team to ensure all aspects of the resident’s care are addressed.
Maintaining current knowledge and competency in nursing practice.
Participating in the mentoring and orientation of staff.
Considering factors related to safety, cost, and impact in planning the delivery of care.
Providing leadership in the professional practice setting under the direction of the RN.
Demonstrating knowledge and competency in nursing practice.
Occupational exposure to bloodborne pathogens is anticipated, with required tasks involving potential contact with blood, body fluids, or tissues.
