Job Title: Licensed Practical Nurse (LPN) – Part Time
Job Specialty: Long-Term Care (LTC) LPN
Job Duration: 13 Weeks
Shift: Part-Time Evenings (3 PM – 11:30 PM), 2×8 hours one week, 3×8 hours the next week with every other weekend and holiday required
Guaranteed Hours: 24 hours per week
Experience: Minimum of 1 year experience as an LPN
License: New York LPN Licensure
Certifications: BLS (AHA or RQI only)
Must-Have: High School Diploma or GED
Job Description:
– Meet residents’ Activities of Daily Living (ADL) needs as outlined in the plan of care, ensuring a safe and comfortable environment, including hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, and skin care. Complete documentation as per protocol.
– Maintain residents’ environment and safety, including answering call lights and alarms promptly, providing safety mats and low beds, using restraints under nursing direction, maintaining unit cleanliness, and preventing falls. Complete documentation as per protocol.
– Ensure residents’ nutritional needs are met as per the plan of care, including meal delivery, tray preparation, adaptive equipment, feeding, and honoring resident preferences. Complete documentation as per protocol.
– Address residents’ rehabilitative needs according to the plan of care, including range of motion, ambulation, transfers, proper body alignment, and use of assistive devices. Complete documentation as per protocol.
– Report residents’ rights, refusals, and concerns to charge and/or supervisory nurse in a timely manner. Promptly report changes in residents’ physical, emotional, and psychosocial status to charge nurse.
– Reasonably anticipate occupational exposure to bloodborne pathogens. Required tasks may involve potential contact with blood, body fluids, or tissues.
Skills:
– Minimum one year of experience in a long-term care (LTC) or certified nursing assistant (CNA) capacity.
