Job Title: Registered Nurse (RN) – Home Health

Shift: Day – 5×8 – 9am – 5pm
Guaranteed Hours: 40 per week
Experience: Minimum two (2) years in a HOME setting within the last fifteen (15) years
License: Current RN Licensure in New York State
Certifications: BLS (required before start date)
Must-Have: Recent experience conducting comprehensive assessments of patients

Job Description:

– Perform initial in-home pre-evaluation of patients, evaluating health status, strengths, care needs, and preferences to guide individualized care plans.
– Review consumer medical documentation and health referral forms as necessary.
– Enter evaluation data into electronic form and transmit as required.
– Document any concerns, conflicting information, or issues during evaluation.
– Discuss health care options, supports needed, service vendors, and waiver options with consumers.
– Initiate communication between Call Center and eligible consumers for enrollment in managed care health plans.
– Collaborate with interdisciplinary care team members to improve care coordination and facilitate service delivery.
– Collect quality review data and required documentation for outcome measurements.
– Identify opportunities for health promotion and illness prevention.
– Maintain knowledge of community resources, payor requirements, and network services.
– Meet all standards outlined in annual performance criteria and bonus template.
– Perform other duties as assigned by Regional Nurse Manager or project management.
