License: RN License (New York)
Certifications: BLS, OASIS

Must-Have:
Certified Home Health Care Experience
EPIC or Netsmart Experience
Current NYS driver’s license
Critical thinking skills
Strong interviewing, clinical assessment, and communication skills
Ability to exercise initiative and independent judgment
Ability to build and maintain professional relationships
Work independently with minimal supervision
OASIS experience

Description:
The Registered Nurse Case Manager is responsible for developing and revising treatment and discharge plans in collaboration with patients, families, and healthcare providers.
This position involves completing and submitting OASIS assessments, reassessments, and discharge documentation within defined timeframes.
The RN will process orders and notify physicians about patient needs and changes in condition while ensuring compliance with care plans.
This role also includes education and consultation with patients, families, and team members regarding disease processes and self-care techniques.
The RN will supervise LPNs to ensure patient health and safety and coordinate the quality of care services provided.
Maintaining respectful interactions with patients, physicians, and referral sources is essential.
The case manager will support care management initiatives aimed at meeting individual health needs and ensuring quality outcomes.
They will also complete clinical records for compliance with regulations.
Flexibility to visit patients based on need is required.
The RN will monitor quality indicators and customer satisfaction while pursuing efforts to reduce avoidable costs and errors.
Participation in quality assessments and performance improvements is expected.
The RN will contribute to a professional environment through ongoing education, self-development, and adherence to key performance indicators.
