Job Title: LPN/LVN – Med/Surg
Location: Klamath Falls, OR – 97601
Shift Timing – Nights – 3*12hrs 
Start Date: 06/09/2025
Job Type: Contract (13 Weeks)

Pay Median: $1650/week – $1750/week

Job Summary:

The LPN will support patients who are medically ready for discharge but experiencing delays due to non-clinical barriers. Working under the supervision of an RN, the LPN will assess, implement, and monitor care plans, provide patient education, and collaborate with interdisciplinary teams to support efficient discharge processes.

Key Responsibilities:

Patient Care:

  • Perform focused assessments and implement care based on the RN’s plan

  • Assist with discharge planning and patient readiness

  • Monitor and evaluate patient response to care interventions

  • Assign and supervise tasks delegated to CNAs or UAPs as needed

Documentation:

  • Accurately record assessments, interventions, and patient responses in Epic

  • Complete documentation in compliance with hospital policies and regulatory standards

Patient Education:

  • Reinforce discharge plans and instructions for medication and follow-up care

  • Provide basic patient and family education
    SKILLS

    • Acute care nursing

    • Discharge planning processes

    • Patient assessment and focused clinical observation

    • Basic nursing procedures (vitals, wound care, IV monitoring, etc.)

    • Effective communication and documentation (Epic EMR)

    • Basic patient and family education

    • Time management and prioritization

    • Team collaboration and task delegation
