Job Title: Licensed Practical Nurse

Location: Hallock, MN

Pay Rate: Up to $45/hour or $1800/week

Position Summary:
NO PENDING LICENSE ACCEPTED – MUST HAVE LICENSE AT TIME OF SUBMISSION

Shift: 1- LPN– Day/Eve shifts- 40 hrs, 8’s and 12’s
Ratios: 1:20-25

2 References Required at Time of Submission
Requires: 1.5 or 2 yrs. of LTC/Critical Access within the last year

First-Timers Accepted: Yes
Weekend REQ: E/O Weekend Required
Certs REQ: BLS/ACLS – IV Certification is a plus
Is On-Call REQ?: N/A
Will this traveler need to float between like units?: Traveler may need to float upon request
Open to accommodating block schedule?: No – Needs to work on what the center needs
Locals Accepted: Yes

Pending License Accepted?: MUST PROVIDE PROOF OF LICENSE AT TIME OF SUBMISSION
RTO Restrictions: RTO ONLY APPROVED AT TIME OF SUBMISSION. WILL NOT APPROVE AFTER OFFERED. Cannot take every holiday off

ANY CALL OUTS MUST BE GIVEN WITH AMPLE NOTICE, MORE THAN 24 hours PRIOR

Special Requirements: Alternating/Flex Shifts – If Subbed To Position, Provider is Agreeing to a Flex Schedule with a guarantee of 40hrs per week

Active, unrestricted certification/licensure as an LPN in Minnesota State.
