Job Description

Job Title: Registered Nurse Case Manager

Experience: Minimum of one year as a clinical nurse in a hospital; preference for nine months of experience in home care
License: RN License (New York)
Certifications: BLS, ACLS

Must-Have:
– Certified Home Health Care Experience
– EPIC or Netsmart experience
– OASIS experience

Description:
Develop and revise treatment and discharge plans in collaboration with patients, families, physicians, and care teams.
Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition according to defined timeframes.
Process orders and notify physicians of patient needs and changes in condition.
Provide an environment that promotes respect for patients, their privacy, confidentiality, and property.
Initiate appropriate preventive and rehabilitative strategies.
Provide services requiring specialized nursing skills.
Consult with and educate patients, families, and team members regarding disease processes, self-care techniques, and preventive strategies.
Supervise LPNs to ensure patient health, safety, and compliance with care plans.
Direct, coordinate, evaluate, and supervise the quality of patient care services provided by the agency.
Foster positive relationships with patients, physicians, referral sources, and others.
Support care management philosophy that includes collaborative planning and case management aimed at meeting individual health and service needs.
Ensure the completion of all clinical records necessary for compliance with legislation and delivery of seamless service within established timeframes.
Maintain current knowledge of available community-based services.
Demonstrate flexibility and willingness to visit patients based on need rather than geography.
Ensure problem resolution at the point of contact.
Attend patient care conferences for patients assigned to care.
Monitor results and take corrective action on quality indicators, including customer satisfaction.
Understand the impact of nursing functions on patients and overall operational success.
Pursue efforts to reduce avoidable costs and errors.
Respond to findings of Quality Management reviews/audits and ensure resolution of potential issues.
Participate in peer review and Quality Assessment and Performance Improvement as assigned.
Maintain productivity requirements.
Ensuring appropriate visit authorizations are in place, reflecting medical necessity for care in the home setting.
