Description:
Coordinates and directs the provision of home nursing care according to physician’s orders.
Care is based on agency policies and procedures through the competent application of the nursing process.
Responsible to Clinical Manager.
Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Must be willing to travel up to 75 miles for home visits.
Periodic office meetings are required.
Must have reliable transportation and current driver’s license.
Agency-required liability insurance is necessary.
Average number of patients per day is 5-8.
Each type of visit and activity holds a different point value.
Contractor will be assigned between 5-10 patients per day.
Types of cases include Start of Care, Wound Care, and Follow-up Visits.
Weekend and on-call requirements vary by location.
Weekly productivity expectations are around 30 points.
Documentation must be completed within 24 hours of patient visits.
Delayed and incomplete documentation may impact follow-up patient care.
No guaranteed hours are provided with this contract.
All overtime must be pre-approved.
Contractor must accept every patient on their schedule regardless of distance.
Local candidates are accepted.
Mileage reimbursement is provided at the IRS standard rate.
