Hours per Shift: 12 hours
Experience: 24 months minimum of current Level I Trauma PCU experience required
License: Active state license
Certifications: ACLS (AHA), BLS (AHA)

Must-Have:
Candidates with an Arizona home address on their driver’s license will not be considered.
All candidates submitted must be pre-closed and ready to accept.
No Nursys hits are allowed.

Description:
Candidates must include a copy of their driver’s license in the submission.
Veteran all locations to the travelers and add all experienced departments the candidates are willing to accept/work in.
Ensure all requests are noted in the highlights section and the form is 100% accurate.
Start dates should be on or very quickly after the order start date listed.
All requested time off (RTO) should be listed and must be less than 7 days.
RTO will not be approved in the first week of the assignment or during holidays.
All facilities the traveler is open to working in should be listed in the submission.
Indicate what states the traveler is interested in (e.g., any state).
All shifts the traveler is open to working on must be listed.
All departments the traveler is willing to work in must be checked on the cover page.
Let us know if the traveler withdraws or is no longer available.

Travel experience is preferred.
All submissions must include an active license verification that is less than 30 days old.
Skills checklist specific to the discipline must be less than a year old.
Certifications must be active at the time of submission.
On-call schedules will be determined by the facility.
Weekend schedules will be determined by the facility.
