Position: RN – Case Manager
Location: Bronx, NY 10461
Shift: 9:00 am – 5:00 pm
Duration: 13 Weeks(Contract)
Guaranteed Hours: 40 hours per week
Pay: $2750 – $3100/Week

Overview

We are seeking a dedicated Case Manager to join our team. The ideal candidate will play a crucial role in coordinating patient care and ensuring the smooth transition between healthcare providers.

Duties

  • Coordinate patient care by collaborating with healthcare providers
  • Maintain accurate and up-to-date medical records
  • Utilize medical terminology and knowledge of anatomy and physiology to support patient care
  • Ensure compliance with HIPAA regulations in all aspects of medical documentation
  • Work closely with nurses and other healthcare professionals to optimize patient outcomes

Skills

  • Proficient in patient care practices
  • Strong understanding of medical records management
  • Knowledge of medical terminology, anatomy, and physiology
  • Experience working as a nurse is a plus
